What we collect

This website only collects what you choose to send us. When you use the appointment request or contact form, we receive the details you type: usually your name, phone number, email address, a preferred day and time, and your message.

How we use it

Your form is emailed to our front desk so we can call, text or email you back about your appointment or question. We don’t sell, trade or share it with anyone else. The website does not keep a copy of your form after it is sent.

Please don’t send detailed medical information through the website. We’re happy to talk about your health by phone or at your visit, where it’s protected under our Notice of Privacy Practices (the “Protecting Your Health Information” form on our patient forms page).

Cookies and tracking

This website does not use advertising or analytics cookies, and it doesn’t track you across other sites. Your browser may remember small settings, such as whether you paused the animations. The street map on our Location page loads from OpenStreetMap only if you choose to show it.

Links to other sites

Links to Google Maps, Apple Maps, Google reviews and OpenStreetMap take you to sites with their own privacy policies.
